If you intend to travel to the territory of the Autonomous Region of the Azores, you will be required to take a test upon arrival on the island of your final destination, unless you present:
a) Proof, in digital or paper format, of a valid COVID EU digital certificate of testing;
b) Proof, in digital or paper format, of a certificate issued by an accredited laboratory, nationally or internationally, attesting to the performance of a screening test to SARS-CoV-2, carried out by the RT-PCR methodology, within 72 hours before departure.
c) Proof, in digital or paper format, of a certificate attesting that the holder of the subject to a rapid antigen test listed in the list drawn up by the European Commission based on the Council Recommendation of January 21, 2021, concerning a framework for the use and validation of rapid antigen detection tests for a COVID-19 and the mutual recognition of test results in the EU within the last 48 hours with a negative result.
You can also, from the questionnaire, request to schedule (subject to confirmation) to screening test to SARS-CoV-2, with one of the contracted laboratories, and attach the result afterward.
As of July 1, 2021, the obligation to perform SARS-CoV-2 diagnostic tests does not apply in the following situations:
• Passengers who present the valid COVID Digital EU Vaccination Certificate
• World Health Organization International Certificate of Vaccination
• Vaccination certificate issued by third countries, which must contain the information listed in the current Government Council Resolution. The validity of these certificates is only recognized if their holders have been inoculated with a vaccine against COVID-19 approved by the European Medicines Agency

To submit your entry into the Azores, you must fill out the questionnaire and submit the result of your test within 72 hours before the beginning of your trip,
if you have a valid certificate of vaccination or recovery, you do not need to fill it out the present questionnaire.
